The role of the Anticoagulation
Center is to provide safe, effective management of a variety
of anticoagulation medications in both inpatient and outpatient
settings. By empowering the patient and family through
education, our goal is to decrease the incidence of bleeding/clotting
complications, as well as provide a dedicated, standardized
approach to their care. We serve as a resource for health
care providers, patients, and community centers to facilitate
appropriate transitions for interventions of all patients
undergoing anticoagulation therapy throughout the health
care continuum. For example, if a patient currently taking
a blood thinning medication is scheduled for upcoming
surgery, he or she may need to stop taking the medication
or descrease the amount taken for a time before, during
and after surgery. Our staff helps patients through these
types of transitions.
Anticoagulation Therapy
is the close monitoring of blood levels for patients
taking blood thinning medications, such as Coumadin
(Warfarin). Though a highly effective medication for
many cardiac patients, Coumadin may cause some adverse
reactions if not monitored closely. When used incorrectly
or without regular blood tests, Coumadin can cause serious
side effects. Too much of the drug can cause patients
to bleed easily, sometimes internally. Conversely, too
little of the medication may result in blood clots to
the legs, lungs, or other areas of the body.
Highly skilled nurse practitioners with many years
of cardiology experience are happy to provide patients
with anticoagulation therapy to ensure the best possible
outcome. They are:
- Carol Patrick, RNFA, MS, CCRN, CNAA
Director, Anticoagulation Management Services
- Kathleen Reed, RN, MS, ANP-C
