APPOINTMENTS

The Dolan Family Health Center is open:

HOURS
Monday, Tuesday, and Wednesday: 8:00 A.M. to 5 P.M.
Thursday: 11:30 A.M. – 8 P.M.
Friday: 8:00 A.M. to 4 P.M

APPOINTMENTS
Appointments are preferred for routine visits. Walk-ins are welcome for sick visits.

To schedule an appointment, or for more information,
call (631) 425-5250. Patients are asked to call in advance if they will be late, or to cancel or reschedule an appointment.

Please try to arrive at least 30 minutes early for your appointment. When you arrive, check in with the receptionist. There are many important parts to your visit both before and after seeing the doctor. These may include financial screening, chart preparation, nurse interview and laboratory work. Generally, your first visit will take longer than follow-up visits. Please allow enough time for your appointment at the Dolan Family Health Center.

DIRECTIONS

FROM LIE

LIE to exit 51 North (Deer Park Ave.)
Continue North on Deer Park Ave. Stay in left lane because road will fork just past Dix Hills Fire Dept. Bear left at fork. Cross over Jericho Turnpike (Deer Park Ave. will become Park Ave.). Continue for 6 miles to Pulaski Road. Make right onto Pulaski Rd. Health Center is 4th Bldg. on left hand side, 284 Pulaski Road.

FROM NORTHERN STATE PARKWAY

Northern State Parkway to exit 42 North (Deer Park Ave.)
Continue North on Deer Park Ave. Stay in left lane because road will fork just past Dix Hills Fire Dept. Bear left at fork. Cross over Jericho Turnpike (Deer Park Ave. will become Park Ave.). Continue for 6 miles to Pulaski Road. Make right onto Pulaski Rd. Health Center is 4th Bldg. on left hand side, 284 Pulaski Road.
