Case Management
POSITION: HOME CARE LIASION
HOURS: Full - Time (with weekend rotation)
EDUCATION: Registered Professional Nurse, BSN or Bachelor's in related degree required.
DESCRIPTION: The Home Care Liasion has knowledge of Huntington Hospital's discharge planning process to accept and refer patients appropriate for skilled home care services. Under supervision, formulates and implements a Home Care Plan in conjunction with the physician, case manager, social worker, patient, patient's family and discharge planner. This individual will be responsible for timely provision/flow of clinical information to/from third party payers and CM's to ensure certification/approval of post discharge services and ensures referrals are made to ancillary care providers to improve efficiencies.
REQUIREMENTS: RN holding a current NYS license. Education listed above. Two years of home care intake, community health service experience in an HMO, home care agency, or insurance company required. One year minimum of medical/surgical nursing experience required. Five years of related health care experience required. Effective interpersonal skills, with strong written and verbal communication skills. Ability to transition into a Case Management role with training.
