POSITION: ADULT NURSE PRACTITIONER ANTICOAGULATION MANAGEMENT SERVICES
HOURS: Per Diem
EDUCATION: Registered Professional Nurse. Adult Nurse Practitioner license. MSN required. Adult Nurse Practitioner certification required. Successful completion of an Anticoagulation Management Certification program preferred.
REQUIREMENTS: All education listed above. Anticoagulation Management experience preferred. Good Customer Service skills are a must.
JOB SUMMARY: The Adult Nurse Practitioner works in collaboration with the Director of Anticoagulation Management Services utilizing established practice protocols, and evidence-based clinical guidelines. He/she demonstrates clinical competence in history and physical examinations, nursing assessment and patient education, diagnostic testing and communication of findings/results and other activities related to the effective managment of the patient receiving anticoagulation therapy. He/she communicates among the interdisciplinary team members of the hospital nursing staff, surgeons, anesthesiologists, members of the medical staff and the physician's office staff and rehab centers to ensure a seamless continuum of care. He/she possesses the ability to recognize the etiology, risk factors and clinical manifestations of pathologic thrombus formation, as well as an in-depth knowledge of the pharmacological properties/toxicities of antithrombotic drugs. Facilitation of external regulatory requirements in an effort to promote absence of clot or hematoma formation in a variety of patient populations, as the transition from acute care to the outpatient arena.
