POSITION: TEMPORARY SOCIAL WORKER
HOURS: Full - Time Days (Monday-Friday) with some weekend and holiday rotational coverage.
EDUCATION: Master's Degree in Social Work
DESCRIPTION: Provides assessment and intervention, supportive counseling and crisis intervention to patients and/or their families during patient's hospital stay. Collaborates in timely discharge planning and ensures continuity of medical care via referral for appropriate follow-up services.
REQUIREMENTS: Master's Degree in Social Work. NYS LMSW license required. Experience in case finding, provision of direct services to patients through the case work and/or group work approach. Ability to work collaboratively with multidisciplinary hospital staff as well as with outside health and welfare agencies.
