POSITION: CASE MANAGER
HOURS: Full - Time
EDUCATION: Registered Professional Nurse, B.S. preferred.
DESCRIPTION: In collaboration with the Director for Case Management, physicians and administrative leadership, the Case Manager coordinates patient care and services to maximize efficient and cost effective use of hospital resources and promote high quality patient care within the heathcare continuum. Additional areas of focus include, but are not limited to, achievement of desired/optimal outcomes, promotion of patient self-managed care to prevent readmission and facilitation of timely discharge.
REQUIREMENTS: RN holding a current NYS license. ANCC certification in Case Management is preferred. If no certification is held, it must be obtained as soon as the eligibility requirements are met. Three to five years of progressive experience in medical-surgical or critical care nursing. Case Management experience is preferred. Sense of humor, outstanding interpersonal skills, including team building. Demonstrated ability to think critically, to speak publicly and write well. Demonstrated ability to function as a change agent. Knowledge of changing healthcare requirements. Excellent work ethic. Enthusiasm for hospital mission and vision. Customer Service skills are a must. Applicants who do not meet the above qualifications but feel they should be considered may apply.
