What is a Hospitalist?
Hospitalists are physicians on site 24 hours
per day, 7 days a week, who oversee patients’
healthcare needs while they are hospitalized.
These dedicated professionals work nowhere
else but in the hospital and provide a vital
link in communication between the patients,
their family members and patients’ primary
care physicians. When a physician in the community
decides that a patient requires a hospital stay,
he or she telephones the hospitalist, discusses
the patient’s medical history and current
condition, and faxes any pertinent medical records.
The hospitalist then assesses the patient upon
admission and provides the primary care physician
with frequent updates on any changes in the
patient’s condition as well as all discharge
notes. Hospitalists are available around the
clock should patients and their family members
have any questions. This model of hospital based
care has been shown to improve the quality and
efficiency of health care as well as both patient
and community based physician satisfaction.
When
will I see my primary care physician?
Many primary care physicians refer their patients
to a Hospitalist when their patients are sick
enough that they can no longer be cared for
in an outpatient setting. During your hospital
stay, you will be seen by a Hospitalist who
works with your primary care physician. You
will see your regular doctor soon after discharge
from the hospital.
How does the Hospitalist know about me?
Hospitalists talk to primary care physicians
frequently. At the time of your admission to
the hospital, your primary care physician calls
the Hospitalist and informs him or her of your
condition and sends over records to provide
as much information as possible about your illness.
The process is similar to what your physician
does when he or she refers you to a specialist
for another opinion or for additional tests
and treatment.
During your hospital stay, your Hospitalists
and primary care physician may talk further
regarding your treatment. At discharge your
Hospitalist will communicate with your primary
care physician to discuss your further treatment
needs, help arrange follow-up care, and prescribe
necessary medications. He or she will send your
discharge information to your primary care physician.
How do I or members of my family get in
touch with the Hospitalist?
In order to speak to your Hospitalist doctor,
ask your nurse to page us.
