Once
you are scheduled for surgery, the staff in your
physician’s office will assist you with scheduling
an appointment for pre-surgical testing. Please
try to arrive 15 minutes prior to your appointment
in order to provide us with important Admitting
Department registration information such as your
contacts and insurance provider. Your preoperative
work-up may include laboratory tests including
x-rays and an EKG; a medical history and physical
performed by a nurse practitioner; and a nursing
admission assessment. The entire process usually
take less than 1 ½ hours.
Your physician’s office will give you a three part pre-admission form to
complete. Please complete all the information requested and mail the top copy
to the Admitting Office. Bring the remaining copies with you on the day of your
pre-admission testing. If for some reason you cannot keep your appointment, please
contact your surgeon’s office as soon as possible as it may affect your
planned surgery date.
